In the book, the Chief made several mentions of McMurphy being larger than him.
Even though no one was larger than the Chief, he was a head taller than all of them.
In the movie, there’s only one line, he tells Jack Nicholson
“You’re bigger than I am”
I think it’s a reference to spirit.
As to say “You still laugh”..
This thing hasn’t beat you yet, you haven’t been bested.
And the chief also made several references in the book, about the system working on him.

Except they get really mad when you use racial slurs for fun, or bring up Israel in any capacity at all…
